You should check the payment instruction that was provided by your institution. You may not have to fill the correspondence section.

The key areas are usually beneficiary information and , which i believe you habe been provided with.

Please go with your vaccine card or get a letter from your doctor saying you are pregnant and can't be vaccinated. This was what I was told on the 25th. I couldn't get a letter before my flight took off.I had to reschedule my flight.

I would suggest that you pour over the thread from like page 25. You might find some useful insights.

Otherwise, here's my take
1. If you're not showing, you're not pregnant. It's not new that some people are genuinely unaware at early stages of their pregnancy. You don't need to call attention to your self.

2. If you're showing, carry yourself like a boss. There are no rules against sight seeing while pregnant.

3. If you're obviously showing. Stick to your few weeks tourism story, and don't deny being pregnant

We've had extreme cases earlier in the thread that claimed they were asked to conduct pregnancy test. But these were very rare cases.

I believe if your documentation are complete and reasonable, you won't have any issues. All the best.

You would see several hospitals and their websites. Visit the website on google chrome and let it translate the webpage from Portuguese to English then check for their forms or email and send them a message stating your intentions. Most of the hospitals will respond to you with a quotation. I got this one from ILHA Hospital e Maternidade. You can search them too, they are a top hospital in Florianópolis/SC.
