I have searched everywhere but couldn't get it. I forgot to mine when I made payment in November.

The only option I have now is not forget the 26k I paid and do fresh application.

You can blur your personal information on the form.

I went for capturing today and the guarantor form was not requested for, only those who were ignorant were asked to at least get the data pages of their guarantor's port, as for me I was casually asked what date I paid which was obviously 5th on my forms, then i was asked why i didn't print the guarantor forms out, to which I responded that i forgot to print the forms out but when I went back I couldn't get it to print, and that ended the questions.

I was at the Alimosho immigration office from 8am and up until 6pm I had not done my biometrics, I was only able to finish 2 steps, that is after arranging my file at the servicom desk, it went from there for further confirmation after which it was brought back to me stamped as confirmed, was eventually asked to come back very early tomorrow morning with my file numbered for recognition. They had network issues which slowed down the pace of capturing although the crowd was much too.
Next day:
I finally was able to get it done few minutes to 11am, after waiting for the network to be restored for like 2 hours.

I tried the new automated port application on the immigration website and it was easy and fast.

I also used the NPC website to apply for my birth certificate attestation, and I got it almost immediately.

Has the price now been regularized? Most places in northern Nigeria used to have significantly lower prices than the south. Didn't understand the logic behind it then.

Bro please does that mean I don't need to make photocooy of guarantor port since guarantor form is not required

You are to every able and print them. Those are the hard copies that will represent you inside the file before capturing. They will also scrutinize the documents both the printout and originals before attending to you.

If you need the GUARANTOR FORM and want it to be edited to ur details for a token

Yes, so guys I applied and paid for the port on the 9th of this month.

I captured today at Asaba processing office. I was supposed to capture yesterday but their new head of operations just resumed and was not on seat due to meetings to give approval. So I was told to come today.

Officers were all friendly

Documents that were asked for were

The appointment slip
Application slip that has been vetted already
Payment receipt
Original birth certificate (later returned to me)
Original LG identification certificate ( returned also)

They gave me a port collection slip and said a message will be sent to me to come for pick up of the port.

This was all purely DIY.

I believe once you have all your documents for fresh/reissue, the process will be smooth.

I underwent the capturing process at the Ikoyi port office on Tuesday. The documents listed below align with those I submitted for my port renewal. Additionally, I was required to photocopy a one-paged document, which I filled and submitted along with the other documents.

A brown file was opened for me, and all the documents underwent verification. Subsequently, I proceeded to the capturing room, where my file underwent enrollment. During enrollment, my NIN was cross-checked, and my application was entered into the system. Following the capturing process, I received a brown slip with a unique number.

When I inquired about the expected completion date for my port, the officer informed me that I would need to track the application online. Overall, the DIY process was relatively smooth, and I can confirm that there is no need to pay any officer for assistance with your port application. I left the port office around 12:30 pm.

I will provide updates as necessary, particularly when my port is ready for pickup.

I will advice you to wait till next week Friday. But if u don't mind wasting your tfair, you can go there to check. You should have link up with one officer on the day of capturing in order to help you check if it has be printed before going for pick up.

On NIS page, choose Oyo State as your capturing state, then choose Oyo as your capturing centre.

Oyo town capturing centre is on the first turn to your left as you turn into Aawe road, Oyo town. Ask okada guys at Owode, Oyo to take you to immigration office. It's #500
