If your cousin actually selected "yes" to indicate a past denial, but the officer claimed he selected "no," there might have been an error in submission or miscommunication. He should retrieve his previous DS-160 confirmation page to what was submitted. Probably he couldn't demonstrate strong home ties and weak documentation. Let him work on those.

Yes, your port number will change with anything that you do in your port be it change of name/address/spouse name/port renewal, etc.
