saddens me every time i hear such news..."Most of the deaths and disabilities attributable to childbirth are avoidable because the medical solutions are well known,the challenge that remains is therefore not technological but strategic and organisational" (alvarez et al, 2009). we really have a long way to go and may posterity judge us well in the end-that we did all we could at all times as one involved in providing emergency care, I'll discuss some factors we need to be aware of 1. Where did the woman for her ANC care? if she ed there, then they have all obligation to attend to her once in labour. if not- when it is an emergency she ought to be referred to a specialist hospital due to late presentation and associated maybe unknown negative history such as hypertension etc. 2. Was the ANC care provided adequate and was she educated on the signs of labour and the need for early presentation once she has seen her show or noticed increase in contractions painful or not. then how many ANC sessions did she attend? 3. Our training for nurses and midwives needs to instil confidence in them as to recognise and manage post Partum Haemorrhage.(bleeding after childbirth). we see nurses and midwives that scream "DOCTOR OO" once they see the excessive bleeding and also our young doctors need to simply follow the pathway of management. 4. we all agree that our health facilities are derelict and deficient, but with what we have, some deaths are not justifiable please. FINALLY for prospective parents, once your wife takes in. 1. begin to save monthly towards the childbirth ( as little as 5k or 10k monthly) 2. go on a hospital research- find the one closest to your residence that offers the following a. specialist care- the ANC is run by a O and G consultant. b. here is always a doctor on call at night. c. they have a functional theatre d. have a track record of safe deliveries - shows nurses and midwives are doing well (maybe) 3. be vigilant for warning signs of labor as at the 8th - 9th month 4. have transport ready especially within the 8th and 9th month 5. keep some cash at home within this period also- so if you ned to deposit any sum- you are able to do so even if in part. maternal mortality is preventable guys! 7 Likes 1 Share |

Toksytoks: i agree with some points of yours! but also uraemia doesn't present suddenly...and to our colleague saw the previous night. except he missed the signs. what of uraemic encephalopathy: Chaz b was still coherent till demise. like you said, the doctor shouldn't be bothered with giving fluids-but for me with taking a sample later. a multidisciplinary team is a far shot in this country so..i would have loved the doc to do the basics...and referral on phone to a doctor that was not present is no referral, they could have moved him to a facility where the specialists were on ground, that is after doing all to stabilize him. sending him home that night was also wrong IMO |

Raymondenyi:hello ray, sad you chose to insult yourself rather. as one doctor to another please mind your language and show respect. if you clearly read my points, i simply offered my opinion and did not label or cuss anyone. please if that's all you have to say then don't quote me. our opinions may differ that's why we have clinical conferences but never be rude. the ABC's of emergency management still apply.... A-Airway: did our colleague ensure that Mr Charles maintained a patent airway? B-Breathing: did he ensure that his breathing was efficient? was there need for oxygen even via nasal prongs C-Circulation: did his vitals necessitate augmentation with IV fluids? if not...there was no need battling to set up an IV line just then, except if he wanted to take a sample for blood chemistry. Etizz:hello , not holding brief or talking down on anyone but the doc on night duty should have followed all emergency protocols (if they had any tho) question is... did he have a diagnosis? did he have the necessary skill and equipment to stabilize the patient? did he refer when it was beyond his scope? when he assumed that 11am was late for the senior to arrive why did he not refer immediately? sadly things are done for gain and not in patients best interest. so pained to lose Chaz B one of my mentors like this... 1 Like |

sad that after surviving a kidney transplant, he died due to negligence.... i weep for my country. a state of emergency needs to be declared in the health sector ASAP as Piers Morgan once said "with this deaths we lose a vital part of our common existence, a part of our collective soul has just been lost" ![]() ![]() ![]() ![]() ![]() ![]() as a medic..could easily have been acute abdomen...he could have benefited easily from ission, an Nasogastric tube, a urinary catheter and close monitoring of his vitals through the night, while awaiting blood work and radiology (which should be available 24/7 in emergency centres) lack of emergency protocols is killing my people! the doctor relies on book knowledge and intuition when medicine is both science and an art. recognise the symptoms and then treat or refer if beyond the scope of your practice.
