If it is for the new e port, they need to see the physical slip at the port office. Try and make out time to go collect it before appearing for ur port application

He can fill out ur guarantor’s form and scan back to you.
U will need ur guarantor to have either drivers license or national id plus 1 port picture and also the data page of his own International port. I have not heard of voters card being used before

I can only give detailed response for centres that are currently issuin only the ENHANCED new e port, thats ikoyi lagos, abuja nd port harcourt
For this fresh e port u will need
1. Nin slip for baby
2. Nin slip for parent 1
3. Nin slip or driver’s licence for parent 2
4. Consent letter for baby’s port from parent 1, can be handwritten
5. Consent letter for baby’s port from parent 2, can be handwritten
6. Data page of international port for either parent
7. 2 port photographs
8. Acknowledgemnt slip
9. Payment slip
10. Application form print out.
11. Original birth certificate of child

How to proceed, you go online to the immigration portal , seek out fresh port
Fill out all necessary information till the end.
Have ur baby’s nin number because it will be asked for on the form.
Every body evn 1 day old baby must have nin to obtain new intl prt.
It is free and u can go early to their offices, with ur baby u will be attended to.
Print out the application form before goin on to the payment
Amount shown for new e port 32 pages is 25k, when u pay with ur card its total 26k
Then print out number 8 and 9 on the list.
Then proceed to the pasport office of ur choice with other above listed documents.
They will then tell u how to submit ur documents and day of ur interview/capturing.

For centres with less number, u can do all this in one day. Go also with originals of all documents bcus they must be sighted.

If mother has intl port bearing former name and is different from baby’s surname on birth cert, u should present with newspaper change of name, marriage certificate and court affidavit change of name attached to mother’s data page of intl port with former surname.

I have seen the page days before I paid the 30k istrative charge. In the page, they said 5 working days. Mine had ed more than 5days. That's why I am here seeking advice from some of you who might have gone through the similar route.

No it's not required... Renewed on Monday this week, went with it but the immigration guy said it's not needed...
All u need is 1 port photograph and the expired port that's for renewal

1. marriage certificate
2. nimc slip with the new name you wish to use, that is if it was in your maiden name you need to change it to your new surname. just visit nimc website, pay 500, almost 600 or 700 Sha for remita as the change of name fee. then print out and proceed to any nimc centre to have the change effected.
3. newspaper change of name as well.

If you have all the necessary documents (like the marriage certificate), then its exactly the same process

I was at the Ikoyi port office today, and the requirement for the modification of name are the following:
1. marriage certificate
2. newspaper publication
3. change of name affidavit
4. receipt of payment for name change on NIN.
* for number 4 above, you either pay online totaling like 670-700 or you pay 1000 at the people at the desk, and trust me, those people in charge of this thing take money like water. in my presence, a woman paid 5000 just for registration and they even ask her to come back tomorrow.
i am also going back tomorrow for my modification, i got there quite late today, number shared for people doing NIMC has finished before i got there.

I already ed the application form, the payment receipt and guarantor's form but I have some questions
1) Where will I the acknowledgement form that I have been seeing on this thread
2) How will I know my interview date
3) Which other documents will I carry along to the immigration office with the printed forms
4) Any other advice you will like to give me

U are good to go with the new slip showing ur married name.
At the paspt office u need to show the marriage certificate, newspaper publication and affidavit.

Go back to ur application usin ur applicatipn id on the portal and . U will be able to print out ur acknowledgemnt form. It’s there.
It looks alike to the payment slip, the difference is that it is labelled as acknowledgement slip.
U will need ur guarantor’s form, filled stamped and sworn abi signed at the high court by commissioner of oaths. That shud be 500 naira
Then guarantor shud also give u one port photo, data page of his intl port data and then his or her driver’s licence or voter’s card.

Documents for you are
Letter of identification from Local Government/State of origin or residence or National Identity Card OR Letter of introduction for employees of government as a Nigerian citizen stating Service Number, Length of Service and Designation/Rank. The letter must be accompanied with a valid staff identity card;
Completed port application form and 2 port sized photographs (port photographs endorsed at the back by the Guarantor);
Completed Guarantors’ form duly signed by a Commissioner for Oaths attached with Photocopy of Data page of Guarantor’s Nigerian eport and one port sized photograph of the Guarantor;
Marriage certificate, where applicable;
Birth Certificate (certificate issued by the National Population Commission shall be required from applicants whose date of birth is with effect from Dec 1992) or Age Declaration attached with one port-sized photograph and duly endorsed by a Commissioner for Oaths
Acknowledgement slip and evidence of payment
