U dnt av to fill d first except u've gone to Poly nd u got in thru DE. D second, check ur applicatn record slip, look up jst above ur port nd look 4 sth lyk 'state itted set by system ' or so. D date beside it is ur ission date. Den write ur course on d nxt line.

Anyways u could take it to the cafe guys in shopping complex in uniben, tell em u wanna type the two letters... They know what to do, dey already av d format in deir system...

Note : the guarantors letter is to be signed by a parent or ur Guardian (d person paying ur fees), d acceptance letter to be signed by you accepting the course given to ya.
