Jonjohnson: Print the following Payment Acknowledge slip Payment receipt Application form 2 port photographs Your expired port A letter to the comptroller General CGI Muhammad Babandede MFR requesting for a port reissue A photocopy of your expired page( data page and the last page) A photocopy of your payment receipt Your NIN slip Take all this documents to your selected port office Go straight to the servicom desk, they will direct you to the officer in charge of submission, a 3 pages form will be provided for you to photocopy and fill.
