I had the same situation, all my certificates carry a different name from my present name and the name on my port.

An affidavit and newspaper declaration should suffice

1 . Can I use it for my biometrics that way without any explanation or do I need an affidavit.

Do I go ahead and start my application as soon as my funds are ready or wait for my wife and baby to get there PoF ready. I feel the school start date is close n don’t want to be late. All children must see a clinician who’ll decide if they need a chest x-ray. Children under 11 will not normally have a chest x-ray.

You must take your child to an approved clinic(IOM) and complete a health questionnaire. If the clinician decides your child does not have TB they’ll give you a certificate. Include this certificate with your child’s UK visa application

I don't want to judge but it seems the bank statement was altered. It is evident that UKVI will always bank statements. I don't know for other docs but that bank statement verification is a must.
