Pls I need help!!!

I'm a fresh graduate who wants to further his education in Canada. Meanwhile, I already applied for a family sponsorship about 3 years ago but after medical examination there was no any other response from them. So we decided to try the student route.

Now, the problem is that the date of birth on my transcript is different from the one on my international port. (Age on NIN, Birth certificate and international port are the same and correct but the one on jamb, waec and transcript are different). And the school I'm applying to in Canada needs my transcript (unofficial or official) to complete the ission process. Pls what can I do?
