For renewal at Shagamu port.

Appointment slip (1 copy)
Payment receipts (2 copies)
Standard port application slip( 2 copies)
Expired port
2 port photographs
Brown file or you buy there (#500)

Take your birth certificate or age declaration (if born after before 1992) just Incase.

my friend who captured in April at Abeokuta has collected port, though she paid for fast track.

In addition to the above documents.

Gaurantors form,signed at an high court.
Consent letter from the parents
State of origin certificate (I think the parents own should suffice)
Birth certificate must be from NPC
